Privacy Policy

JK Services AZ, LLC is committed to protecting the privacy of members, families, support coordinators and website visitors.

Information We Collect

When you use a form on this website — such as the general inquiry form, the tour/enrollment request form, the community experts form, or the employment application — we collect the information you choose to provide, which may include your name, email address, phone number, and the details of your message or request.

Our general web forms are designed to avoid collecting detailed protected health information. Sensitive identity information for employment (such as Social Security number or date of birth) is collected only later, through a secure hiring and onboarding process after the initial application stage.

How We Use Your Information

We use the information you submit to respond to your inquiry, schedule tours, process referrals, evaluate employment applications, and communicate with you about JK Services programs and services. We do not sell your personal information to third parties.

Cookie Policy

This site uses a small amount of browser storage and, only with your consent, analytics cookies. Consent is the legal basis for Google Analytics processing. Necessary storage supports security and remembers your privacy choice; it does not require consent.

Necessary storage: Your accept/reject choice, the notice version, and its date are kept in your browser's local storage for up to 180 days. Pages containing forms may also use a temporary session cookie to protect submissions against spam and unauthorized requests.

Analytics cookies: If you choose “Allow analytics,” Google Analytics 4 (measurement ID G-KTGS2VS02M) may set _ga to distinguish visitors and _ga_KTGS2VS02M to maintain session state. We limit their lifetime to 180 days. Analytics may process pages viewed, approximate region, and device/browser information. Google Signals and advertising personalization are disabled.

Before you choose, Analytics storage is denied and the Google Analytics script is not loaded. “Use necessary only” is offered alongside “Allow analytics.” When you allow Analytics, the first permitted Analytics event records the notice version you accepted so JK Services can document the consent workflow. You can withdraw consent at any time through “Cookie Settings” in every page footer; withdrawal immediately disables collection and removes accessible Analytics cookies from this site. Your choice is requested again after 180 days or when this notice materially changes.

Google may process Analytics data in the United States and other countries under its applicable contractual and transfer safeguards. Review Google's Privacy Policy and Google Ads Data Processing Terms for details.

External map content: Pages displaying an embedded Google Map connect your browser directly to Google. Google may receive your IP address and browser information and may store or read cookies under its policies.

You can also block or delete cookies in your browser settings. Blocking the temporary security cookie may prevent forms from working correctly; rejecting Analytics does not limit site access.

How We Protect Your Information

We take reasonable technical and administrative measures to protect the information submitted through this website, including secure form handling and restricted access to submitted data.

Member Rights

Members and their representatives have rights regarding their information under applicable Arizona DDD requirements. Contact our office directly with any questions about your records or rights.

Changes to This Policy

We may update this policy from time to time to reflect changes in our practices. When the cookie notice changes materially, the site will ask for your choice again.
